You will be required to sign the following:

• Consent for treatment
• Authorization to bill insurance

Please remember to bring with you at the time of registration the following items:

• Social Security Card
• All active insurance cards
• Picture ID
• If you have an Advance Directive such as a Living Will or Durable Power of Attorney for Healthcare, please bring bring a copy to place in your hospital records.
